Our Health and Wellness Home Visits utilize meticulous assessment of potential problems and preventing new ones while we maintain our active senior’s optimal independent living.

 

→  Initial First Visit is 2 hours Comprehensive Visit

→  Weekly Visits for 1 hour for Maintenance Visits

→  Additional Visit as needed  (Drainage Check/Wound Management, etc)

2-hour Initial Visit consists of:

• Medication Management

• Vital Signs/Blood Sugar Checks

• Nutrition and Dietary Assessment

• Safety Assessments

• Health and Wellness Assessment

1-Hour Weekly Visits Consist of:

• Vital Signs and Blood Sugar Checks

• Comprehensive Assessments

• Safety Implementation

• Coordination of Care

• Nutrition and Dietary Support

• Health and Wellness Prevention Education

Retainer Fee/Payment are to be collected upon signing up and on the first day of the Month.  We do NOT take insurance but we accept Major Credit Cards and Flexible Spending Account/HSA.
